Intake & Eligibility

If you are a resident of Adams County and believe you may be eligible for the programs available at North Metro Community Services, please call the Resource Coordination Department at (303) 457-1001 and request an intake.  If you are interested in eligibility information and you have an Aurora mailing address, you will need to contact the following agency for help: Please call Developmental Pathways at (303) 360-6600.

Our Intake Coordinator will begin the process to determine if you meet the eligibility criteria.  In Colorado a developmental disability for anyone over five years of age is defined as a disability that:

  • Occurs before the person reaches 22 years of age
  • Substantially impacts the person's daily life
  • Is caused by mental retardation or related conditions; for example, Cerebral Palsy, Autism, Down Syndrome, Epilepsy or other neurological conditions
  • Impairs the person's general intellectual functioning: IQ of 70 or below
  • Significantly limits daily living skills in two or more areas

Eligibility

Children from Birth to Age 3 Years
These children and their families are offered a wide variety of services through our Early Beginnings program, and, fortunately, State of Colorado funding has been immediately available to meet many of their needs.

Children from Age 3 to 18 Years
Children in this age category are eligible for participation in the Family Support Services Program (FSSP) which provides resource information and funding for therapies, medical and dental co-pays associated with the child's disability, and necessary equipment not paid for by Medicaid or private insurance. Emergency Respite Care funding may also be requested by parents. On a yearly basis, parents of children in this age group are asked to complete either a Family Support Plan (currently accessing FSSP) or an Annual Update document, returning it to North Metro.

Re-determination of Eligibility at Age 6 & Age 14
State of Colorado law requires that the eligibility for continuing services for children at age 6 must be re-determined. School documents, such as a child's Individual Education Plan (IEP) and test results for IQ (Intelligence Quotient) test results are reviewed by our Eligibility Committee. A decision is then made about continuing eligibility. At age 14, a child's eligibility is also reviewed.

Adults 18 Year of Age and Over

This wide age group of consumers may be active in the Supported Living Services Program or the Comprehensive program or may be on our adult waiting lists. Active consumers work with a Resource Coordinator and receive case management services which include at minimum a yearly meeting to develop their Individualized Plan (known as the IP). Those on waiting lists are sent an Annual Update to complete and return to North Metro, this document helps our agency keep track of changes in a consumer's situation.
