Resource Coordination is a service of North Metro Community Services that assists individuals and families with eligibility determination and assessment, individual plan development, information and referral, assistance in applying for Medicaid Waivers and monitoring of services and supports. Resource coordinators are responsible for determining eligibility for and coordinating a variety of services for the adult population, including:
- Support Services – an array of services for people who live at home or on their own.
- Comprehensive Services – funding for day and residential programs.
