Adams County SCC Activity Fund
Restrictions on direct charging people receiving Supported Community Connection (SCC) services for activities combined with low reimbursement rates has led to limitations on the type and scope of activities. These regulations have also led to oversaturation for sites that have little or no cost. North Metro Community Services, Inc. (NMCS) is establishing a fund to support paid activities for people receiving SCC Day Services and that reside in Adams County. The intent of this funding is to provide resources to agencies so that they may expand opportunities for community integrated activities in either the DD Waiver, SLS Waiver, or through State SLS.
Who is Eligible?

This activity fund is available to all Program Approved Service Agencies providing SCC services to eligible people living in Adams County. This funding is being provided with Adams County Mill Levy dollars and can only be used to support people living in Adams County. Funding provided is to be used exclusively for paid SCC community activities and not to support general operations or in house programming.
Application for Funding
Data will be submitted via an excel workbook. Individuals need to be listed separately on the workbook to maintain an accurate count of the unduplicated number of people receiving SCC services that fit the eligibility criteria. It is not necessary to list by name, you may list as person #1, person #2, and so on. You will need to provide an estimated # of SCC hours served annually by person and the program (DD, SLS, or State SLS). The home address must also be included for each person. This address needs to be verified as being in Adams County. This level of reporting will allow an equitable distribution of funds for each person. You will be required to submit forms in excel format. Once the workbook is received you will receive the submitted information in docusign to certify the submitted information. You will need to email the excel file with complete data to grants@nmetro.org.
Deadline for submission to receive funding will be June 30, 2025
Applications received after that date will not be accepted.

Annual Allocation
and Reporting

This program and funding is expected to start July 7, 2025 and provide funding based on availability of $250,000 annually. This program will provide for 13 months of funding and $250,000 through June 30, 2026. Funds will be allocated based on a pro-rata share of the total units of SCC provided to Adams County residents. Distribution of funding will occur at the beginning of each month for 12 months, starting in July 2025.
Reporting on use of funds will be required annually in July for the prior period ending June 30th. The reporting will request use of funds by activity type, number of people supported and total cost of activity. Expenditures of funds should be equal to the amount of funding received for the reporting period. This program is expected to continue annually and funding for the ensuing year may be applied for simultaneously with the prior year reporting.
