Community First Choice (CFC)

Community First Choice (CFC) is a new Health First Colorado (Medicaid) program. It offers expanded home and community-based services (HCBS) to people of all ages who need long-term care, helping them live safely and independently at home.

WHO IS ELIGIBLE?

You may qualify for CFC if you:

  • Are enrolled in Health First Colorado
  • Meet the institutional level of care
  • Have an assessed need for CFC services

Or, if you’re currently receiving services through an HCBS waiver and use at least one waiver service per month.

How to Access Services

  • Current HCBS members: Talk to your case manager during your renewal (Continued Stay Review).
  • New applicants: Contact your local Case Management Agency
Services Available for Community First Choice (CFC)

PERSONAL CARE

Personal Care services are available when they are not Health First Colorado benefits. Personal Care services provide a range of assistance to enable participants to accomplish tasks that they would normally do for themselves. Services may include skin, nail, hair, and mouth care, bathing, shaving, dressing, eating, walking, exercise, transferring, positioning, toileting, and respiratory care. Services may also include grocery shopping, attending routine medical appointments, and meal planning and money management.

HOMEMAKER

Homemaker Services are general household activities provided in the home of an eligible member to maintain a healthy and safe home environment when the person ordinarily responsible for these activities is absent or unable to manage these tasks. Services may be hands-on help, performing a task for the member, or prompting them to perform a task. Homemaking services may include the following: routine light housecleaning, dusting, vacuuming, mopping, bathroom and kitchen cleaning, laundry, bed making, and meal preparation.
